Choose SIGN UP on the lower right of your phone or tablet screen.
When asked for your health insurance, select FLORIDA HEALTH CARE PLANS from the list.
Enter the information from your FHCP insurance card on the next screen.

What is the Cost?

After creating an account in Doctor on Demand and selecting Florida Health Care Plans as your insurance carrier, provide your information located on your FHCP insurance card, you will see then see your reduced* copay amounts for both:

Medical Visits
Psychology Visits

*For Health Savings Account (H.S.A) compatible High Deductible Health Plans, the deductible must be met first before the copay applies. Contact Member Services to see if your deductible has been met.
