MyChart Overview

MyChart a secure online patient portal that will help FHCP patients access their health information. By creating a FHCP MyChart account you can schedule appointments, access test results, message your care team, receive appointment reminders, get on a waitlist to be seen sooner and more!

Appointment scheduling

With MyChart, you can schedule or request medical appointments. You can also view your appointment history and details about each one. When you arrive at the office, you can minimize wait times by using MyChart to check in for your appointment.

Video Visits are also an option to receive convenient care with MyChart. Depending on your reason for visit, FHCP providers can provide you care with no in person appointment required!

• Log into MyChart > Select Schedule an Appointment

• Next, choose your reason for visit. You will then have the choice to choose between ‘In Person’ or ‘Video.’

Note: this will only show if video visits are allowed for the particular reason for visit you selected.

Reminders through Hello World

MyChart enables you to receive appointment reminders. The MyChart app will ping you when it’s time for your appointment, when you have new test results or when your physician messages you back

• Log into MyChart > Go to the menu in the top left corner > under Account Settings, select Communication Preferences
• On the right side of the page, under Details, you can edit your notification preferences by clicking the drop down arrow.
• Select Advanced settings to update your individual preferences for each topic:

eCheck - In

eCheck-in prior to your visits to save time by answering questionnaires and verifying or updating information!

Sharing Hub / Friends and Family Access

In the sharing hub you can share access to your own health information, whether it be a list of immunizations you've received for school or work or giving a family member long term access to your MyChart account.

• Log into MyChart > Select the Menu in the top left corner > Under Sharing, select Sharing Hub

• From the Sharing Hub, scroll down and select the Manage friend and family access option to invite others to see your record as well as manage whose records you can see:

Share Everywhere

With share everywhere you can grant one-time, limited access to your health information in MyChart. You may need to use this feature when seeing a new doctor, chiropractor, physical therapist, etc.

• Log into MyChart > Select the Menu in the top left corner > Under Sharing, select Sharing Everywhere.

From here, you can request a one-time use share code to give to your provider. They can then enter the code on www.shareeverywhere.comto see your health record summary and even add their own notes to your chart.

Find an Extended Hours Care Center

Let the Extended Hours Care Clinic's (EHCC's) know you are on the way to secure your spot in line! You can use this feature for urgent visits or for walk in lab visits.

• Log into MyChart > Select the Menu in the top left corner > Find an Extended Hours Care Center

Message your Care Team

Have a question for your doctor? With MyChart's messaging feature you can securely chat with your care team about test results, visits, medications, or a brief medical question.

• Log into MyChart > Select the Menu in the top left corner > Under Communication, select Ask a Question.

Paying Balances

Pay off balances, view payment history and set up payment plans in the Billing Summary.

• Log into MyChart > Select Billing Summary
