FHCP has adopted a population health focus aimed at helping our members get healthy, stay healthy, and manage existing chronic diseases. As part of our Population Health Management (PHM) strategy, we evaluate the needs of our entire membership to connect you with PHM programs and services tailored to your needs. Our PHM programs include:
- Annual Flu Shots
- Diabetic Retinal Exams
- Breast Cancer Screening
- Virta – Diabetes and Weight Management
- Emergency Room Utilization
- Antidepressant Medication Management
Conditions and Diseases General Wellness Guidelines

Member Education & Resources
FHCP offers members several programs and services focusing on prevention, early detection, and management of chronic health conditions at no cost. There is no limit to the number of programs or sessions members can engage in. Flexible scheduling options are available to accommodate members’ needs.
The FHCP team is dedicated to listening to and serving our members. Our Registered Dietitian Nutritionists (RDN), Registered Nurses (RN), and Certified Diabetes Care and Education Specialists (CDCES) help empower individuals with tools, skills, and techniques necessary to manage their health.
