Case Management - Coordination of Care

This is a free program offered to members who may benefit from assistance with coordinating their medical, psychosocial, and financial needs. Working with members and their physicians, Case Managers can provide the education and resources needed for members to better understand and comply with their plans of care. Other programs are available to meet your health care needs including in-home medical management and community resources coordination. A Case Manager will help you determine which programs are right for you. FHCP partners with ActiveHealth Management to provide case management services to our members.
